Insurance Agreement & Terms and Conditions of Liability 
It is understood that the liability of Viti Freight Systems Canada Inc. and its consolidation agent will not exceed the amount of $100.00 (Canadian Dollars), or declared value of goods inclusive of freight charges, whichever is lower. Said liability is limited to total loss only, unless otherwise declared in writing and corresponding insurance premiums paid. The company is only liable for no more than what is a basis for insurance claim. All claims and settlement of claims must be made in writing and received by an authorized representative in Surrey, BC Canada. In case of physical loss or non-delivery of said shipment to ultimate destination, the basis of payment for settlement of claim is the total declared value, to include freight/brokerage, and delivery charges as stated on the receipt. Consignors are required to state specifically in writing the declared value of the shipment.

Shipper-Consignor/Recipient - Consignee/Owner(s) Agreement  
This is to certify the following : (1) that I/we am/are the sender(s)/consignor(s) of the above shipment; (2) that the above shipment is the true and correct description of the goods contained in this shipment being sent to Fiji; (3) that there are no undeclared ,restricted, illegal or banned items, including firearms, ammunition, illegal drugs, combustible goods, included in this shipment.

Letter of Certification and Authorization 
I/we as owner(s), shipper(s)-consignor(s)-Recipient(s)/Consignee(s) of the above shipment hereby : (1) duly authorize Viti Freight Systems Canada Inc. and its accredited breakbulk/consolidation agent, to represent me/us on m y/our behalf as my/our custom broker to expedite, physically receive, arrange custom entry and clearance to include all technical requirements and associated details, arrange payment of duties and taxes and other related charges; (2) bind(s) together with our authorized representative, to reimburse the customer broker of the increase rate, apart from all charges mentioned earlier, upon delivery or pickup of the shipment and finally to deliver upon request, should there be changes in rates and additional taxes and other tariff duties imposed;(3) understand and agree that any misdeclaration of items, particularly contraband and restricted articles in the shipment, shall be at the risk of shipper-consignor/recipient-consignee, and that the forwarding agent and custom broker shall not be held responsible and liable whatsoever;(4) understand and agree that perishable/breakable shipments are shipped at my/our risk;(5) understand and agree that all contents of the above alternate straight bill of lading and the attached detailed packing list and export declaration is an integral part of this agreement/authorization/certification;(6) understand that unless declared in writing and corresponding insurance premiums are paid, maximum liability of Viti Freight Systems Canada Inc. and its consolidation agents is $100.00 (hundred dollars) and is limited to total loss only. 
I/we, as owner(s), shipper(s)-consignor(s)-Recipient(s)/Consignee(s) of the above shipment release, Viti Freight Systems Canada Inc. my/our said custom broker, and my/our forwarding agent from any responsibility and liability for all technical delays or all the above mentioned brokerage services/agreement , and/or suspension on the release of our cargo shipment in general from the authorities concerned, for reasons and circumstances beyond the control of Viti Freight Systems Canada Inc., my/our said broker and my/our forwarding agent/representative.
I/we as owner(s), shipper(s)-consignor(s)-Recipient(s)/Consignee(s) hereby agree that in case a law suit arises from this letter of certification and authorization, the venue shall be in the city of Surrey,BC Canada.
